At Liberated Mind Therapy, your privacy and confidentiality are deeply valued. This policy outlines how your personal information is collected, used, stored, and protected in compliance with the Personal Health Information Protection Act (PHIPA), 2004, the standards of the College of Registered Psychotherapists of Ontario (CRPO), and other relevant privacy laws.

1. Collection of Personal Health Information

We collect personal information that is relevant to your therapy services, including:

  • Contact details (e.g., name, address, phone number, email)

  • Date of birth and emergency contact

  • Records of your visits to Liberated Mind Therapy

  • Care you received during those visits

  • Payment and billing information

This information is used solely to provide quality psychotherapy services and comply with legal and professional obligations.

2. Use and Disclosure of Personal Health Information

Your personal health information will be used and disclosed for the following purposes:

  • To provide you with appropriate and personalized psychotherapy and counselling services

  • To maintain accurate clinical records

  • To communicate with you as needed

  • To obtain payment for your treatment (from WSIB, your private insurer, or others)

  • To plan, administer and manage our internal operations

  • To comply with legal, regulatory, and ethical obligations‍

3. Consent & Limits of Confidentiality

All information shared in therapy is kept strictly confidential unless:

  • You provide written consent to share it

  • There is risk of harm to yourself or others

  • A child (under 18) is at risk of harm or neglect

  • Records are subpoenaed by court

  • Disclosure is required by a regulatory body such as the CRPO

We will always try to discuss any necessary disclosure with you in advance, unless legally prohibited.

4. Your Rights

You have the right to:

  • Access your clinical records and personal health information

  • Request corrections to inaccurate information

  • Withdraw consent (except when disclosure is required by law)

Requests can be made by contacting your therapist in writing.

5. Storage and Security

Your records are securely stored:

  • Electronically in Jane, a PHIPA-compliant encrypted platform with password protection and limited access

  • Physically, if applicable, in a locked and secure location

Only authorized professionals have access to your records.

6. Use of Electronic Communication

  • Emails are not fully secure, and should be used only for administrative purposes (e.g., scheduling)

  • Video sessions are conducted using secure, encrypted platforms

  • Clients are encouraged to use a private space and secure internet connection during virtual sessions

7. Sharing Information with Others

Your information is never sold or shared with third parties for marketing. Information is only shared with other professionals (e.g., family doctor, psychiatrist) with your informed and written consent — unless required by law.

8. Supervision and Consultation

Therapists may engage in supervision or consultation to enhance service quality. In such cases, your identity is protected, and no identifying details are shared without consent.

9. Record Retention and Disposal

Your records will be securely retained for:

  • 10 years after your last session

  • If you were under 18 at the time of your last session, 10 years after your 18th birthday

After this period, records will be permanently and securely destroyed in accordance with professional and legal standards.

10. Changes to this Privacy Policy

We may change our privacy policy from time to time. The most current version of this policy is available on our website or at our office.

​‍If you are concerned about our privacy practices, you are encouraged to contact our Privacy Officer. If your complaint is not resolved to your satisfaction, you may contact the Information and Privacy Commissioner of Ontario.
